When Central Georgia Health Network was preparing to take on financial risk for the care of their patients, their physicians had limited experience with population health management. So, CGHN developed a comprehensive practice transformation program for 1000+ providers that addressed the educational needs of a network unfamiliar with value-based care. In this practical, hands-on case study, CGHN will explain how they prepared a large, diverse clinically integrated network to deal with changing payment models. Ultimately, they enabled independent physicians and hospital-employed providers to partner with hospitals to improve financial and clinical outcomes.
