In L.A. County, 38% of the population is under Medi-Cal, California’s Medicaid. Clinicians in the L.A. County safety net are stretched to capacity and the state’s population continues to swell. Safety net practices tend to be reactive and fragmented environments where population health management is minimal. Considering these factors, ensuring that Medi-Cal patients in L.A. County receive the care they need, has become exceedingly difficult. This session will demonstrate how L.A. Care Health Plan's Los Angeles Practice Transformation Network (LAPTN), is increasing the use of evidence-based tools and data across the safety net as part of the CMS Transforming Clinical Practice Initiative. So far, LAPTN has helped its clinicians produce tens of millions of dollars in cost savings and met numerous quality improvement targets. The session will include final results from the 4-year program, a practice use case presented by a LAPTN quality improvement coach, and explore how other organizations can replicate L.A. County’s success.
