Healthcare has long struggled with care standardization, one of the reasons why measuring the care experience is the cornerstone of HCAHPS (Hospital Consumer Assessment of Healthcare Providers and Systems). The ubiquity of smartphones creates an opportunity for providers to standardize care coordination and patient journey mapping across patients, providers, health systems, and caregivers. In this session, South Shore Health System (SSHS), located in Weymouth, MA, details how it has been able to increase HCAHPS scores for its OB/GYN department with the help of a mobile app that provides uniform, current information to expectant mothers to manage appointments, track fetus development, learn what to expect in the hospital, discharge instructions, and more. For the four quarters ending March 2019, the hospital rating score increased 38%, and the care transition score increased by 70%. The session will also address mobile app deployment strategies that hospitals can employ in their own organizations.
