Effective communication is the cornerstone of patient safety and interoperable healthcare. The Joint Commission’s national patient safety goal, to “improve the effectiveness of communication among care providers,” challenged healthcare organizations with how to best accomplish this goal. Additionally, outcome-based Meaningful Use regulations made it imperative to closely evaluate discharges versus readmissions, when a shortened length of stay could result in readmission penalties. Having initially implemented their discharge desktop to meet meaningful use guidelines, Halifax Health looked beyond meaningful use to see its full potential, from improved collaboration to reduced lengths of stay. An effective discharge process involves deployment of an organizational, multidisciplinary, team-based communication structure including a mobile voice-activated system with secure texting, embedding best practice content in EHR desktop, and leveraging transparency with the power of analytics.
