As a participant in New York State’s DSRIP program, OneCity Health must reduce avoidable hospital use by 25 percent within five years for its 750,000 attributed lives, and improve performance in 56 metrics. After failing to meet three major milestone goals in Measurement Year 3 (MY3), the organization faced a six-month window to achieve DSRIP MY4 metrics. With millions of dollars in financial incentives at stake, the OneCity team launched an ambitious effort to procure a data platform capable of taking an analytic deep dive into the issues for quick and targeted quality improvement interventions. Credible data, benchmarking tools, and constant performance feedback helped achieve buy-in from stakeholders. Applying a data-driven approach, OneCity Health quickly uncovered opportunities to reduce avoidable admissions and readmissions for asthma patient populations.
