Implementing standardized EHR content, including order sets, have the potential to produce more favorable clinical outcomes for patients. However, standardizing care and aligning EHR functionality across multiple distinct platforms over a large geographically diverse national health system poses significant challenges. Moreover, assessing the impact of change efforts itself poses unique challenges. As part of a larger change management effort, we implemented standardized EHR content across a large multi-hospital national healthcare organization with the aim of rapidly assessing and treating patients at risk for sepsis. We found that the use of standardized order sets was strongly and independently associated with a reduced mortality, LOS, and cost after adjusting for potential confounders. The lessons learned from this experience highlight the need for leadership driven, tech enabled, and flexible, yet coordinated efforts, across multiple leaders and internal stakeholders to achieve success.
