Spurred by the creation of the federal Physician-focused Payment Model Technical Advisory Committee, the newest innovation in value-based care is the development of physician-focused alternative payment models (APMs). Until now, most APMs have emphasized improvements in hospital care, but physicians are advancing inventive APM approaches to support more accurate diagnosis and treatment plans for patients before complications in their conditions require emergency visits or hospitalizations. These APMs are also utilizing innovative and interoperable health IT solutions to facilitate collaborative care. This session will describe real-world examples of APM proposals designed to help patients better manage chronic diseases and prevent exacerbations, enhance the speed and precision of diagnoses for symptoms or conditions, and improve the process of selecting initial treatment plans. It will also discuss how practices can choose APMs that will support their efforts to improve patient outcomes while lowering health care spending.
