Telemedicine Home Monitoring Solution Drastically Reduces CHF/COPD Patient Re-admissions

Cincinnati, Ohio/Berkeley Heights, NJ - CHI Health at Home, one of the largest healthcare systems in the country, is piloting a comprehensive remote monitoring platform (HomecareVitals) designed and developed by Chenoa Information Services to reduce patient readmissions.  Based on the results, CHI Health at Home is preparing plans to implement HomeCareVitals system wide to support its mission in delivering quality, compassionate care to every patient they serve.

The pilot was implemented with a control group of Congestive Heart Failure (CHF) and COPD patients that had an average readmit rate of 35%. Within two months of deploying HomecareVitals the control groups readmit rate was only 3.8%.

HomecareVitals is an elegant telehealth remote monitoring app that allows the patient to ‘bring their own device’ eliminating the need for expensive and administratively burdensome hospital owned tablets. Patients are provided a complete kit with the necessary devices for blood pressure, pulse oximeter and temperature. 

All data captured is automatically and securely transmitted back to the CHI Health at Home's clinical team, allowing for real-time interventions for high-risk patients. During the pilot there were multiple instances of patients being readmitted due to the intervention of the CHI clinical team. Here are just some of the patient testimonials from the pilot:
