Vice President of Clinical Integration and Population Health

Rowena Bergmans is currently serving as VP of Clinical Integration and Population Health for Western Connecticut Health Network (WCHN). Her responsibilities include overseeing the Physician Hospital Organization (PHO) whose membership includes Danbury, Norwalk and New Milford Hospitals along with 881 physicians. In addition, she is overseeing the development and implementation of WCHN accountable care strategies in an effort to align community needs, operations and business models with changing healthcare policy and funding streams. She joined the network after spending several years as a consultant for Norwalk Hospital.
Prior to her consulting role at Norwalk Hospital, Rowena served as Vice President of Medical Affairs for Clear One Health Plans of Bend, OR where she participated in creating the vision and strategy for the company while overseeing all health related services and programs. Rowena has been heavily involved in developing community solutions to address the current challenges facing our care delivery system. She has worked collaboratively with local and State governments, hospital systems, providers, and community members on programs that include inappropriate emergency room usage, integration of physical and behavioral health services, patient centered medical homes for Medicare and Medicaid beneficiaries and developing a multi share product, a community health improvement plan that provided affordable and comprehensive health care coverage to uninsured Central Oregonians.
