The impact of healthcare reform continues to influence provider and payer strategies, particularly as it relates to improving care coordination and care transitions through reduced readmissions and operational efficiency. The move from volume to value has introduced new payment models, making alignment among acute care providers, ACOs, payers and LTPAC providers paramount in order to deliver value-based care. Data transparency facilitates alignment--predictive data analytics enable providers and payers to build and manage ‘smart’ LTPAC networks and improve care coordination across the entire continuum. This session provides a specific case study from LifeBridge Health in Maryland, a health system who is using data analytics to form strategic partnerships with LTPAC providers and drive outcomes-based care. Achieving data transparency through shared analytics enables business and clinical leaders to identify high-risk patients, avoid adverse events, solve for difficult placement issues, mitigate risk and optimize outcomes in LTPAC.
