Creating alignment between hospitals and the independent physicians they contract with is a challenge made all the more difficult in the world of alternative payment models.  Join us to learn how one community-owned regional health system has successfully addressed the problem, creating a collaboration that focuses on population health, a shared desire to improve quality, and maintain continuity of care.  The speakers will share lessons learned, results achieved to date, and offer solid recommendations for how you can emulate their success.
