Description

Population health management promises to deliver value and improve health outcomes by analyzing data captured via EHR systems. Major health systems are implementing population health programs, but they face IT challenges – namely, the lack of structured data within a patient’s longitudinal clinical history, and the challenges of aggregating and analyzing data to better understand what impacts health outcomes. How can programs such as ConCert by HIMSS define a better path towards leveraging structured data to achieve population health management?
