On the continuum of care, treatment does end with a hospitalization or an office visit. Information technology can enable improved home-based care. In this session, HIMSS Davies Award winner Parkland Hospital (Dallas, TX) will share how technology is used to facilitate the administration of outpatient parenteral antimicrobial therapy. This technology-enabled process produced a 47% lower readmission rate, saved 27,666 patient days in the hospital, and saved $40,000,000 for Parkland. Based on most recent studies, only half of adult patients with hypertension are effectively managed. Davies Award winner Lanai Community Health Center (Lanai City, HI), will demonstrate their model for patient-centered, IT enabled home-based care featuring remote blood pressure monitoring is a more effective method for diagnosing hypertension and enables improved hypertension control for the residents of Lanai.
