Description

Fueled by the convergence of forces such as new rules, regulations and programs from the Affordable Care Act to MACRA; a need to bend the cost curve for providers and consumers; and increasing market competition on cost, quality and value; our healthcare system is in transition from a volume- to value- based system while moving from disease and acute care to wellness and chronic care management focus. This session will provide current status of the change to value, and an overview of current regulatory, technology, and market drivers impacting key healthcare systems.

Learning Objectives:

  • Compile strategies to define and identify patient population attributable to your system including stratifying levels of risk in the population
  • Analyze the true cost of care from a clinical activity to episode levels, and direct and indirect personnel/patient care costs
  • Identify a standards-based platform as it applies to health care information, the main standards that are pertinent, and use cases for organizations striving for value-based care
  • Describe key tenants to an effective dashboard build as it relates to population health management strategies
