Multimodal Delivery of Patient Events to Providers

Care coordination, a key aspect of accountable care, requires notifying the right clinicians about significant patient events in real time. Northwell Health will describe how it uses emails, Application Programming Interfaces (APIs) and simple applications to notify providers about patient events such as admission to an emergency department (ED), discharge from the hospital based on patient risk and test results received after patient is discharged. It is critical to deliver notifications in the providers’ workflow, i.e., into the electronic health record (EHR).
Illustrate the complex environment health systems must navigate to coordinate care among providers using different EHRs
Demonstrate the value of API calls that allow providers to save discrete data and information directly into the EHR
Discuss the future of healthcare information technology and care coordination with widespread adoption of APIs for data exchange and interoperability
