IT-Enabling Population Health: A Journey, Not a Destination

Providence Health & Services (PH&S) created the Population Health (PH) division in recognition of the growing percentage of its revenue tied to at-risk contracts for defined populations of patients. Within the PH division, the Office of Population Health Informatics (OPHI) was created to manage population health data coordination, quality measurement, advanced analytics, predictive modeling and IT collaboration. The OPHI also optimizes the ability of Providence’s HIT applications to support population health management. To enable effective IT support for population health management functions, the OPHI assessed the current people, processes and technology supporting population health across the five states Providence serves, selected technology to fill critical gaps and developed a roadmap to move forward with population health informatics initiatives. This presentation describes the informatics journey to date with a focus on the lessons learned and recommendations.
