Glycemic Control in the Hospital

February 21, 2017 — 10:00AM - 11:00AM EST
Orange County Convention Center

Session ID:
83

Description

Faced with a need to identify clinical opportunities to reduce length of stay, Hawai‘i Pacific Health chose improvements in glycemic control as a likely target. Although most published strategies are around reduction in hypoglycemia, our interest was in improving the normoglycemia rates for our insulin dependent patients. We implemented changes in the EHR, integrated our glucometers and implemented a lot of workflow improvements that did result in improved length of stay for this patient population. There were many lessons learned along the way which will be shared during this session.

Learning Objectives:

  • Describe the components of an effective program to improve glycemic control in hospitalized patients
  • Assess the value of reduced length of stay to an integrated hospital system preparing for value based payments
  • Discuss the metrics used to evaluate clinical and nonclinical success in this program
  • Identify challenges and lessons learned during the implementation and maintenance of a glycemic control program for hospitalized patients
