Flexible Health IT Platform for Engaging with At-Risk Populations

A Flexible Healthcare IT Platform that enables rapid design and implementation of care management programs for at-risk cohorts with targeted and customize interventions is presented which includes the following main components: A Business Rules Engine (BRE) with the ability to rapidly author and deploy business and clinical rules (without changing application code) that help identify a cohort of patients and compute risk scores that enable stratification of these patients for outreach. A Business Process Management (BPM) Engine with the ability to rapidly configure, refine and deploy BPM flows that implement a set of actions and interventions for patients enrolled in a clinical program along with the sequence and conditions under which they are delivered. An approach to configure data elements and UI layout for specific cohort and outreach interventions based on the definition of the BPM flow and a configuration tool that helps associate data elements with UI locations enabling the implementation and refinement of UI layouts without changing application code. A Health Information Exchange Instance is leveraged to aggregate patient data from all facilities in a standardized data model and formal; and to generate real time clinical events that can be used to trigger intelligent rules and BPM flows for Care Management.
