Expanding Real-Time Notifications for Care Transitions

Rhode Island Quality Institute leveraged the infrastructure of the statewide Health Information Exchange to deliver real-time notifications for care transitions from/to ED and hospitals for providers' attributed patients. Notifications are presented using multiple platforms, including direct messaging, integration into the EHR and real-time dashboards, depending on the technology and care management workflow capacity of each provider/ACO organization. Launching this capability required that we extend our existing legal agreements with providers, modify our core consent engine to include all patients in providers' panel, collaborate with providers to receive their patient lists, partner with Epic to integrate notifications in EHR, create real-time dashboards, and ensure that provider organization workflow would be improved to adopt the tools. Join us for a review of our experience and learn about the challenges and benefits of enabling care coordination for providers participating in commercial, Medicare and Medicaid ACOs.
