Description

Electronic consultation and referral (eCR) has improved access to specialty care, enriched primary care-specialty care provider communications, and enhanced care coordination. This session will discuss practical tools and approaches to implement and evaluate an eCR system. Participants will acquire better understanding of organization-specific adoption considerations, as well as Quadruple-Aim-focused evaluation methods for improving cost and efficiency, population health and clinical outcomes, patient experience and provider experience. Presenters are eCR experts from the University of California, San Francisco Center for Innovation in Access and Quality (CIAQ) and Center for Excellence in Primary Care (CEPC). In 2005, CIAQ led eCR implementation and evaluation to serve San Francisco’s safety net population, now one of the most mature eCR systems in the country. CIAQ and CEPC now partner to deliver technical assistance on planning, implementing and evaluating eCR.

  • Compare approaches to eCR system development and adoption based on organizational resources, interest and demographics
  • Identify various domains of evaluation using the Quadruple Aim framework, with specifics on the conceptual and logic model, metrics, suggested data sources and stage of assessment
  • Design short and long term evaluation plans and templates for individual organizations and systems
