Success under value-based reimbursement models requires payers and providers to work jointly on shared objectives; it is no longer enough for providers to focus on quality while payers focus on cost. From the unique perspective of Community Health Plan of Washington (CHPW), a provider-owned health plan, attendees will learn how CHPW and Washington State provider organizations have developed an innovative shared strategy from a pilot focused on member attribution to a comprehensive program that leverages a combined claims and clinical dataset to enable quality improvement initiatives and drive increased revenue through clinically-enhanced risk scores. The presentation will outline a roadmap on how to: • Enable transparent and effective data-driven quality improvement programs; • Empower care teams to target patient outreach and manage documentation to improve risk score accuracy; • Align financial incentives between the plan and providers; • Deliver a ROI through cost and revenue improvements.
