The population health movement is early in its journey, however a degree of success has already been achieved. This success has produced new and changing ideas, policies, payment models, learnings, and opportunities. This session summarizes achievement to date, highlights lessons learned, and explores key drivers that have impacted healthcare’s transition to value.
