Connecticut Hospitals Drive Policy with Geospatial Analysis

As a group of independent but affiliated Connecticut hospitals, the Value Care Alliance (VCA) knew they needed to invest in EHR and claims data aggregation and analytics to successfully transition to value-based care but the VCA was surprised by just how powerful that unified data could be in action.
Geospatial analysis enabled the VCA hospitals to hone in on rising risk patients across the state, providing new visibility into the strength of community-based resources to manage and support those patients. Attendees will learn about:
