Description

This capstone session will provide examples of one state's healthcare transformation journey as it successfully applies the Triple-Aim goals to at-risk Medicaid non-emergent ED utilization populations. Through examples from the state’s deployment of LaEDIE (the Louisiana Emergency Department Information Exchange) attendees will learn how providers are better equipped to provide coordinated, personalized care when they are given access to real-time data, and understand how tools support opportunities to manage complex member interactions such as transitions from ED or hospital.

Learning Objectives: 

  • Describe examples of how care is and outcomes are transformed using access to real-time data
  • Discuss effective strategies for care management, patient outreach and long-term care improvement through coordination among care providers
  • Identify how savings are attained when data are used to identify at-risk patients and address trends that lead to higher ED utilization, particularly of a non-emergent nature
  • Apprise the role of managed care partners in the value-based program and share lessons learned on ways to expand to all payer groups
