This session will explore the strategies Orlando Health is leveraging to efficiently manage the health of its vast population and thrive under value-based billing, including building a clinically-integrated network. The specific tactics and tools the organization employed to unlock patient data across all clinical systems, ensure its primary care providers were recognized as NCQA Level 3 Patient-Centered Medical Homes, and streamline key functions such as building registries, identifying care gaps and engaging patients will be identified. As a result, this session will detail the results and benefits realized, as well as offer attendees best practices they can leverage at their own organization. This session will also share details of the current phase of the initiative, which includes identifying the most urgent chronic and catastrophic cases, supplying the data needed for critical interventions and simplifying documentation.
