"The shift form fee for service to value based reimbursement is accelerating, and Population Health Management is emerging as a means of streamlining care, reducing costs, and increasing patient satisfaction. Healthcare organizations need to adapt to this continuously changing environment by adjusting their business models and acquiring or developing capabilities to support these new value based care models. A lot of confusion and uncertainty persists in the industry about what the population health management capabilities should be. The HIMSS Clinical and Business Intelligence Committee’s Population Health Task Force has developed a Population Health Management model that can guide organizations in the selection and implementation of the appropriate capabilities for their specific value based care model based on payment model and patient population.
