Care planning and coordination of care delivery over time and across multiple settings has long challenged the health care community. The spur of health information technology adoption and automation of clinical and administrative processes is slowly beginning to facilitate coordination, collaboration, and continuity of care amongst provider groups, patients and payers. However, lack of standardization across technologies and processes remain a significant barrier to the development and exchange of longitudinal person-centered care plans.
